By completing this form, you will register your child for WOW for the 2025-2026 school year. This form must be completed by a parent/guardian and is valid through the end of May 2026. Please note: ALL of our WOW Volunteers have completed Ministry Safe training, received Ministry Safe certification and passed a thorough background check. We have taken precautions to ensure the safety of your child when he/she is at Center Ridge First Baptist Church (hereafter "CR1"). Permission Form & Liability Waiver

I, the undersigned, give permission for my child listed above, to attend Center Ridge First Baptist Church (hereafter "CR1") on Wednesdays to participate in WOW (Worship on Wednesday - for K-5th grades). I understand that personal injury can and may occur to my child, and I hereby authorize CR1, and any appointed church volunteer, to seek and consent to emergency medical attention for my child as needed, and I further agree to be liable for and to pay all costs incurred relating to such medical attention. I hereby release CR1, its employees, agents, and volunteers, from all liability, claims, demands, causes of action and possible causes of action whatsoever arising out of or related to any loss, damage, injury (including death) that may be sustained by my child while participating in or traveling to and from this event. I give permission for my child to ride in any vehicle designated by CR1, its employees and adult volunteers, while participating in and traveling to and from this event. I agree to accept full responsibility, financial or otherwise, for any damage my child may do to the property of CR1, any properties visited on outing, other's personal property, or vehicles used for transportation. The following behavior will not be tolerated at any CR1 gathering: *possession or use of alcohol, tobacco, or illegal drugs, *possession or use of weapons, including pocket knives, firearms, and fireworks, *inappropriate sexual behavior. Violation of these standards, or being knowingly in the presence of others violating these standards, can and will result in consequences deemed appropriate by the pastor, student pastor, and advisors of CR1, including, but not limited to, being sent home immediately at the participant's expense.
